Using forms in Google Docs saves a lot of time when trying to collect information from numerous people. After we watch the above we're going to create a sample Google doc together.

Then you're going to create one of your own.
Think of a theme (Name your file that theme)
Make sure you ask the following first....

Name:
Age and or Grade:
8 other questions (use a variety of types of questions)
